GP Out Of Hours Service 

Background

GP and patientWe provide GP out of hours services to patients who are registered with a GP within the Bath and North East Somerset, Swindon and Wiltshire clinicial commissioning areas.

Founded by two local GPs in 2004, we have been delivering clinical services ever since. Our Integrated Urgent Care service describes a vision to deliver responsive, streamlined and patient-centred services ensuring people are managed by the right service, first time.

Commissioned to deliver Out of Hours (OOH) Primary Medical and Dental Services (triage only), calls are passed to us from NHS 111, usually with a disposition assigned. Our experienced team actions an appropriate response which may include an appointment at one of the Primary Care Centres across the area, a home visit by an experienced clinician or self-care advice.

Call handling and triage of dental calls is undertaken by NHS 111. Responsibility for treatment rests with us and is provided through our partnership with DHU Healthcare

 

Our Clinical Leadership Team

Doctor's carThe importance of clinical leadership in setting the direction of an organisation, developing its culture, ensuring the delivery of high quality person-centred care and maintaining effective governance is well documented.

Our Executive Management Team includes clinicians from a wide variety of backgrounds, general practice, nursing, the ambulance service and the allied health professions.

These roles provide clinical leadership to the workforce.

Each area of service delivery also has its own Clinical Lead with responsibility for the day-to-day management of their team. This ensures that we maintain high clinical standards whilst facilitating the development of each member of the team.

 

Our Out Of Hours Team

We have a large, highly skilled multidisciplinary team which prides itself on delivering an outstanding service to our patients and their families. We use a combination of GP, Advanced Nurse Practitioners and Prescribing Pharmacists to deliver our face-to-face OOH service.

Most of our GPs are local, working in daytime practice in Bath and North East Somerset, Swindon or Wiltshire and its neighbouring counties. Our Advanced Nurse Practitioners all have advanced clinical assessment, diagnostic and prescribing skills in addition to many of the practical skills often required during an OOH consultation.

When you first get through to the NHS 111 Service, you will be assessed by a call advisor.

Opening Times 

Weekdays: 18.30 to 08.00
Weekends: 18.30 Friday - 08.00 Monday
Bank Holidays: 18.30 previous evening – 08.00 on the next working day after the bank holiday

Primary Care Centre Locations:

Attending a Primary Care Centre is by appointment only. We are not able to provide a ‘walk-in service’.

Our clinics are based at various locations across Bath and North East Somerset, Swindon and Wiltshire including Bristol/Bath, Chippenham, Salisbury and Swindon. At the time of your appointment you will be provided with details of the clinic you will be attending.
